Have you seen Bugzilla? Its bug tracking abilities are fairly difficult
to top. At work we evaluated a number of bug tracking systems before
finally deciding to go with it.

There are a few things I don't like about Bugzilla, and a few missing
features that unfortunately are not an easy thing to implement. The
open source world, you see, is sadly lacking in CASE tools.
Bugzilla doesn't hook into any sort of external development system. For
example, at work we use Aegis to handle our software changesets, and
Bugzilla to track bugs and enhancement/feature requests. Correlation
between the two is done manually -- we will add a note to a bug or
enhancement stating "This is being handled in Project.1.5 change 54",
and the corresponding changeset will have a description stating
"Bugzilla #54: Need more heat sinks". Not ideal, but it's about the
best the OS world has to offer so far.
